West Texas Pediatric Dentistry offers our patient form(s) online so they can be completed in the convenience of your own home or office.
Existing Patients
Before using the patient portal, you will need to call or text us at (432) 242-5747 to request your login credentials. Once you log into your patient portal you will be able to change your username and password by going to the setting gear in the upper right-hand corner.
In the patient portal you can, review patient information, view any messages, secure email or any forms you have filled out or signed in the past, as well as sign or fill out paperwork that has been requested.
